My name is XXX and I am applying for a passport. I am filing on behalf of my father, who is currently living in India and unable to travel abroad due to his medical condition.
My father is 65 years old, retired and a critical care patient due to his chronic medical condition. He has diabetes and hypertension, which has caused severe health complications. He was admitted to a hospital and treated for his medical condition for two months last year. After his recovery, he was advised to stay away from long journeys and take utmost care of his health.
My mother passed away two years ago, so it’s been difficult for my father to manage on his own. He has two children, myself and my elder brother. I am 28, unmarried and working in India. I stay with my father in a small, but comfortable home. My job involves travelling a lot for business meetings and client visits, which keeps me away from my father for long hours.
For past few years, my father has expressed his desire to travel abroad, to visit his siblings
and family. In light of his medical condition, we believe that taking a few days off travel will help him in many ways. He's looking forward to seeing his family and going on a much-needed vacation.
