摘要：PROBLEM TO BE SOLVED: To provide a delivery form, only a slip inside which can be easily grabbed and pulled out thereof in the state that the delivery form is attached to a delivery article.SOLUTION: In a delivery form, an attachment slip 11 which is
positioned on the front side thereof when the delivery form is attached to a delivery article, a delivery slip 13 which is separated therefrom after delivery, a support base material 14 which is directly attached to the delivery article, and a peeling base material 15 which protects an adhesive layer provided on the support base material are laminated in this order. The attachment slip 11 and the support base material 14 are adhered to each other in an area where they do not overlap with the delivery slip. The delivery slip 13 is divided into a cutting scheduled part, which is scheduled to be cut, and a binding part, which is directly or indirectly adhered to at least one of the attachment slip 11 and the support base material 14, by perforations M1. In the support base material 14 and the peeling base material 15, perforations M2 and perforations M3 are formed in positions not overlapping with the attachment slip 11 in the direction connecting the two opposite sides to each other. The delivery slip 13 and the support base material 14 are adhered to each other between the side opposite to the binding part and the perforations M2.
