DONATION FORM
We wish to donate the following items to The Salvation Army and receive a Tax
Deduction for this donation. We will:
____  Donate our entire booth of merchandise
____  Donate a portion of our booth merchandise
Please note: Determining the taxable value of the donation is the sole responsibility of
the donor.
I understand that a Salvation Army Representative will be contacting me to secure the
product in our booth, pick up the hard copy, issue a tax receipt, and pack up the items
we are donating.
I acknowledge that I am authorized to donate merchandise to The Salvation Army.
